SHIPPING INSTRUCTIONS

We strongly advise using a trackable shipper such as UPS, FedEx or Priority Mail, as well as insuring your equipment. Prices shown do not include shipping charges, and New York State customers must pay NYS Sales Tax for their jurisdiction.

PLEASE TELL US WHERE THE EQUIPMENT CAME FROM

If we receive equipment without contact information, it will be set aside and not repaired!

If you use our online RMA system, print out the packing slip and enclose it in your package. Otherwise, please enclose the following information:

• Your Business Name and Unit (store) Number
• Owner’s Name
• Shipping Address and Phone Number
• Billing Address and Phone Number
• A detailed description of the problem
• Name and telephone number of someone familiar with the problem
